Who This Is For
More Common Than Most Men Talk About
Erectile dysfunction affects approximately 1 in 3 men over 40 and is far more common than most men acknowledge. It is most commonly caused by reduced blood flow, hormonal factors, psychological contributors, or a combination of these. It is treatable in the vast majority of cases. The most common reason men do not seek treatment is not knowing where to start or not wanting to have the conversation in person. BoldYou makes that conversation private, clinical, and straightforward.
Difficulty achieving or maintaining an erection sufficient for sexual activity
Reduced firmness compared to earlier in life
Inconsistent function — works sometimes but not reliably
Performance anxiety that has become part of a cycle
These symptoms can indicate underlying conditions including cardiovascular concerns, hormonal factors, or metabolic issues. Your provider reviews your full health history as part of every ED evaluation. If low testosterone is suspected your provider may recommend testosterone assessment alongside your ED treatment.
Our ED medications work as PDE5 inhibitors — they block the enzyme that breaks down cGMP, the chemical that relaxes smooth muscle in the penile arteries and allows increased blood flow when arousal is present. Sildenafil and tadalafil are among the most studied compounds in men’s sexual health with decades of safety data. Vardenafil works through the same mechanism with a slightly different pharmacokinetic profile. Apomorphine works through an entirely different pathway — it is a dopamine agonist that acts on the brain’s arousal circuits rather than on blood flow directly.
In plain English: These medications increase blood flow to the right place at the right time when arousal is present. They do not cause erections independently. The combination formulations address both the physical blood flow component and the neurological signaling component — two systems working together for a more complete response.
On-Demand vs Daily — Which Is Right for You
On-DemandTaken before sexual activity. Works within 30–60 minutes depending on formulation. Best for patients who want treatment only when needed.
DailyLow-dose tadalafil taken once daily. Provides consistent background effect. No timing required around sexual activity. Allows for spontaneity. Also used for urinary symptoms related to benign prostatic hyperplasia.
Your provider recommends the approach best suited to your lifestyle and clinical profile.
Formulation Options
Our Clinician-Prescribed Formulations
Your prescribing clinician selects the most appropriate formulation based on your health history, current medications, and clinical profile. Not all formulations are appropriate for all patients. Here is what will be available.

Important Safety Information
We believe in full transparency. Please review this section before enrolling.
Critical Contraindications
Nitrate medicationsDo not take with nitrate medications including nitroglycerin or isosorbide. Combining PDE5 inhibitors with nitrates can cause a severe drop in blood pressure. Your provider screens for this. Disclose all medications accurately during intake.
Alpha-blockersUse with caution if taking alpha-blockers for blood pressure or prostate conditions. Your provider evaluates this interaction during intake.
Recent cardiovascular eventsIf you have had a recent heart attack, stroke, or have unstable cardiovascular disease discuss this explicitly with your provider before starting.
Common Side Effects
HeadacheMost common — related to vasodilation. Usually mild and resolves within a few hours.
Facial flushingCommon particularly with sildenafil-containing formulations.
Nasal congestionTypical with PDE5 inhibitors.
Vision changesRare — temporary light sensitivity or color tinge. Stop use and contact your provider if significant.
Back pain or muscle achesMore commonly reported with tadalafil formulations.
Rare but Important
Priapism — an erection lasting more than 4 hours — requires emergency medical attention. Seek care immediately if this occurs. This is extremely rare at therapeutic doses.
This is not a complete list of side effects or contraindications. Your provider reviews your complete health history and all current medications before prescribing. Accurate and complete disclosure during intake is essential for your safety.
Eligibility
Who Qualifies — and Who Should Discuss Carefully
Good Candidates
Men 21 and older experiencing ED symptoms
No contraindications to PDE5 inhibitors identified during review
Willing to disclose complete medication list — critical for safety screening
Not currently taking nitrate medications
Have an Honest Conversation With Your Provider If You Have
Current use of nitrate medications — this is a safety contraindication
Retinal conditions — rare formulation adjustments may be needed
Age 75 or older — single-ingredient formulations only
This list is informational. Your prescribing clinician makes all eligibility determinations based on your complete health history and current medications.

Is Low Testosterone Contributing to Your ED?
Testosterone plays a meaningful role in libido and sexual function. In some men ED has a hormonal component alongside or instead of a vascular one. If your provider suspects low testosterone during your ED evaluation they may recommend testosterone assessment alongside your ED treatment. Addressing both simultaneously often produces better outcomes than ED treatment alone.

Common Questions
ED Treatment Questions Answered
How discreet is the delivery?+
Your medication ships in plain unmarked packaging with no indication of the contents on the outside. The return address does not reference BoldYou Wellness or any health-related name. Your credit card statement will show a charge from BoldYou Wellness or our payment processor — not a medication-specific description.
Do I need to see a doctor in person?+
No. ED treatment at BoldYou is fully async — no in-person visits required in most states. Your intake is completed online and a licensed provider reviews it within 24–48 hours. Your medication ships directly to your door.
How quickly do the on-demand formulations work?+
Onset varies by formulation. Sildenafil-containing formulations typically work within 30–60 minutes. Tadalafil has a longer onset but also a longer window of effectiveness — up to 36 hours. Your clinician will explain the timing specific to your prescribed formulation.
Will these formulations work if I have tried single-ingredient options before?+
Possibly. The combination formulations work on multiple mechanisms simultaneously — both the vascular blood flow component via PDE5 inhibition and the central arousal component via dopamine activation (in formulations containing apomorphine). Some patients who have not responded fully to single-ingredient approaches respond better to combination formulations. Your provider reviews your history and recommends accordingly.
Is low testosterone related to ED?+
Sometimes. Testosterone plays a role in libido and sexual function. If your provider suspects a hormonal component they may recommend testosterone evaluation alongside ED treatment. See our TRT and Enclomiphene programs for more information.
What if I am on blood pressure medication?+
Disclose all blood pressure medications during intake — this is critical for safe prescribing. Alpha-blockers in particular require careful evaluation with PDE5 inhibitors. Your provider reviews all medications and contraindications before prescribing.
Can I cancel after joining the waitlist?+
Yes. Joining the waitlist creates no obligation. You will be notified when enrollment opens and can choose whether to proceed at that time.
