Here's what happens, in plain words.
A drink releases the brain's own feel-good chemicals (endorphins). That reward is what builds the pull to drink again.
Naltrexone sits on the receptors those chemicals use. The drink still happens, but the "buzz" that drives the next one is muted.
Without the reward, cravings tend to weaken over weeks. Many people find they simply want less.
GLP-1 medicines, like tirzepatide, also act on the brain's reward and craving centers. Early studies, including a small 2025 trial of semaglutide, found people drank less and had fewer cravings. Larger studies are under way, and GLP-1s aren't FDA-approved for alcohol use.
Naltrexone doesn't make you sick if you drink, and it doesn't stop you from getting drunk. It works best with support, which is why coaching is part of both pathways. Never drink and drive.
